What Causes Leg Pain While Standing?

Standing for long periods can leave your legs feeling tired or uncomfortable. For some people, however, the pain goes beyond ordinary muscle fatigue. Aching, throbbing, heaviness, or pressure in the legs that develops or worsens while standing can be a sign of an underlying problem with the veins.

Your veins have the important job of carrying blood from your legs back toward your heart. Because they have to move blood upward against gravity, they rely on small one-way valves and the muscles in your legs to keep blood moving in the right direction.

When those valves become weak or damaged, blood can begin to pool in the legs. The resulting increase in pressure can cause pain, aching, heaviness, swelling, and other symptoms that are often worse after standing for extended periods.

Leg pain while standing can have many causes, however. Muscle, joint, nerve, and other medical conditions can also contribute to discomfort. If your pain is persistent or occurs along with visible veins, swelling, or other symptoms, a vein evaluation can help determine whether venous disease is playing a role.

Why Do Veins Cause Pain When You Stand?

When you stand, gravity naturally pulls blood toward your feet and lower legs. Healthy veins have valves that help prevent blood from flowing backward.

Your calf muscles also play an important role. Every time you walk, your muscles contract and squeeze the veins in your legs, helping push blood upward toward your heart.

When the vein valves are weak or damaged, blood can flow backward and collect in the lower legs. This is called venous reflux.

Standing still for long periods can make the problem more noticeable because the calf muscles are not contracting regularly to help move blood upward. As blood pools, pressure inside the veins increases.

That increased pressure can produce the aching, throbbing, heaviness, or fullness that many people with chronic venous disease experience after standing for a long time.

What Does Vein-Related Leg Pain Feel Like?

Venous leg pain is often described as a dull ache, throbbing, heaviness, or feeling of pressure in the legs. Some people describe their legs as tired or fatigued rather than experiencing sharp pain.

The discomfort may be concentrated around visible varicose veins or may involve the calves, ankles, or entire lower legs.

One of the most characteristic patterns is that symptoms become worse as the day goes on, particularly after prolonged standing or sitting. The pain may improve after walking, changing positions, or elevating the legs.

Some people also experience swelling, itching, burning, cramping, or a feeling of tightness in the lower legs.

What Causes Leg Pain While Standing?

There are many possible causes of leg pain while standing. Vein disease is one possibility, but it is not the only one.

Venous insufficiency: Weak vein valves can allow blood to pool in the legs, increasing pressure and contributing to aching and heaviness.

Muscle fatigue: Standing for extended periods can cause the muscles of the legs and feet to become tired and sore.

Joint problems: Arthritis and other conditions affecting the knees, hips, or ankles can cause pain that becomes more noticeable when you are on your feet.

Nerve problems: Irritated or compressed nerves can cause burning, tingling, numbness, or shooting pain in the legs.

Foot problems: Conditions affecting the feet, including improper footwear or structural problems, can contribute to pain that travels into the lower legs.

Previous injuries: Old injuries involving the muscles, joints, or bones can sometimes cause discomfort that becomes more noticeable with prolonged standing.

Because the causes are so varied, persistent leg pain deserves an evaluation rather than automatically being attributed to poor circulation.

Is Leg Pain While Standing a Sign of Vein Disease?

It can be. Pain that consistently develops or worsens after standing for long periods is a common symptom of chronic venous disease.

The likelihood that veins are contributing to the pain is greater when other symptoms are present, such as varicose veins, spider veins, swelling around the ankles, leg heaviness, itching, or changes in the skin.

Venous pain often follows a predictable pattern. You may feel relatively comfortable in the morning, only to notice increasing aching or heaviness after several hours on your feet.

Walking may actually make the symptoms feel better because the calf muscles help pump blood upward through the veins.

Who Is Most Likely to Develop Vein-Related Leg Pain?

Anyone can develop venous disease, but certain factors increase the risk.

Age: Vein valves can become less effective over time, making venous reflux more likely.

Family history: Genetics play an important role in the development of varicose veins and chronic venous disease.

Prolonged standing: Jobs that require you to stand for much of the day can make it more difficult for blood to return efficiently from the legs.

Prolonged sitting: Sitting for long periods can also reduce the muscle activity that helps move blood through the veins.

Pregnancy: Pregnancy increases blood volume and places additional pressure on the veins in the pelvis and legs.

Excess weight: Additional pressure can make it more difficult for the veins to return blood toward the heart.

Previous blood clots: A history of deep vein thrombosis can damage the valves inside the veins and contribute to chronic venous problems.

How Is Leg Pain Diagnosed?

A vein evaluation begins with a physical examination and discussion of your symptoms. Your provider may ask when the pain occurs, whether standing makes it worse, whether walking or elevating your legs improves it, and whether you have other symptoms such as swelling or visible veins.

Your legs may also be examined for varicose veins, spider veins, swelling, skin discoloration, or other signs of venous disease.

A lower-extremity venous ultrasound can provide a detailed look at the veins beneath the skin. Ultrasound can show how blood is moving through the veins and identify abnormal backward blood flow, or venous reflux.

This information can help determine whether your pain is related to an underlying vein problem or whether another cause should be investigated.

How Is Vein-Related Leg Pain Treated?

Treatment depends on what is causing the pain. If venous reflux is responsible, treating the unhealthy veins can reduce the abnormal pressure contributing to your symptoms.

Endovenous Ablation

Endovenous ablation is a minimally invasive procedure used to close an unhealthy vein that is allowing blood to flow backward.

Using ultrasound guidance, a small catheter is inserted into the affected vein. Energy is then used to seal the vein closed.

Once the problematic vein is closed, blood naturally redirects through healthier veins. The procedure generally requires only a small access point and typically involves significantly less recovery than traditional surgical vein removal.

Sclerotherapy

Sclerotherapy is commonly used to treat smaller varicose veins, reticular veins, and spider veins.

A specialized solution is injected into the affected vein, causing it to close. The body gradually absorbs the treated vein while blood is redirected through healthier vessels.

Sclerotherapy may be used to treat visible veins that remain after the underlying source of venous reflux has been addressed.

Vein Surgery

Some larger or more complex varicose veins may require a surgical approach. Modern vein procedures are much less invasive than traditional vein surgery, and the appropriate treatment depends on the size, location, and condition of the affected veins.

Your provider can determine which approach is appropriate based on your examination and ultrasound findings.

Can Leg Pain While Standing Be Prevented?

There is no guaranteed way to prevent venous disease, particularly when genetics and age are contributing factors. However, several habits can support healthy circulation and may reduce symptoms.

Regular walking and exercise activate the calf muscles, which help push blood back toward the heart. If you have a job that requires prolonged standing, changing positions and taking short walking breaks can help keep blood moving.

If you spend much of your day sitting, getting up and walking periodically can also improve circulation.

Maintaining a healthy weight can reduce pressure on the veins. Elevating your legs when resting may help relieve pressure and swelling, while compression stockings can help manage symptoms for some people.

These measures may improve symptoms, but they do not repair damaged vein valves or eliminate venous reflux.

When Should You See a Vein Specialist?

Consider having your legs evaluated if pain regularly develops or worsens while standing, particularly if it improves after walking or elevating your legs.

An evaluation is especially worthwhile if the pain occurs along with varicose veins, spider veins, swelling, heaviness, itching, cramping, or changes in the skin around your ankles and lower legs.

You don't have to wait until the pain becomes severe. Identifying the cause early can help determine whether treatment is appropriate and prevent symptoms from continuing to interfere with your daily activities.
