Can Running Make Varicose Veins Worse?

Some people with varicose veins can run without any significant symptoms. Others notice that running makes their legs ache, feel heavy, or swell.

If you have varicose veins and enjoy running, you may have wondered whether every mile is helping your circulation or making your veins worse.

It is a reasonable question. Running is a high-impact form of exercise, and it temporarily increases blood flow to the legs. Some runners also notice that their veins seem more prominent after a workout or that their legs ache, throb, or feel heavy after a long run.

The good news is that running does not appear to cause varicose veins, and for most people, having varicose veins is not a reason to stop running. In fact, regular physical activity is generally beneficial for venous circulation. The calf muscles act as an important pump that helps move blood out of the legs and back toward the heart.

However, there is an important distinction between making the veins themselves worse and making the symptoms of existing vein disease more noticeable. A person with underlying venous insufficiency may find that running aggravates aching, swelling, heaviness, or other symptoms even though running is not necessarily causing the underlying vein problem to progress.

Understanding that distinction can help you decide whether you should continue running, modify your workouts, or have your veins evaluated.

What Causes Varicose Veins in the First Place?

To understand whether running can make varicose veins worse, it helps to understand why varicose veins develop.

Your leg veins have one important job: they carry blood back toward your heart. Because blood has to travel upward against gravity, the veins contain small valves that help keep blood moving in the right direction.

When those valves become weak or stop closing properly, some blood can flow backward and pool in the veins of the legs. This is called venous reflux.

Over time, the increased pressure can cause veins to enlarge, twist, and become visible beneath the skin. These are varicose veins.

Genetics, age, pregnancy, prolonged standing, obesity, and other factors can contribute to the development of venous disease. Varicose veins are therefore not simply a consequence of exercising too much or exercising incorrectly.

This is important because it means that running isn't typically the underlying cause of the problem.

So, Can Running Make Varicose Veins Worse?

Usually, no—not in the sense that running causes the vein valves to deteriorate.

There isn't good evidence that recreational running causes existing varicose veins to progress. In fact, physical activity is generally encouraged for people with chronic venous disease because contracting the muscles of the lower legs helps promote venous return. Guidelines for chronic venous disease recommend physical exercise as part of management, and evidence suggests exercise can improve calf-muscle function, ankle movement, and aspects of venous circulation.

Running has an additional advantage: it repeatedly activates the calf muscles.

Think of the calf muscles as a secondary pump for your leg veins. Every time your calf muscles contract while you walk or run, they compress veins in the lower leg and help push blood upward.

So while running can temporarily increase pressure and blood flow in the legs, the muscle contractions involved in running can also help blood move back toward the heart.

The bigger question is what happens after the run—and how your legs respond to the exercise.

Why Do My Varicose Veins Look Worse After Running?

This is one of the reasons people become concerned.

You may finish a run, look down at your legs, and notice that the veins appear larger, darker, or more prominent than they did before your workout.

That does not necessarily mean you have damaged your veins.

Exercise increases blood flow to working muscles. Your blood vessels also respond to changes in temperature and circulation. After a run, your veins may temporarily become more noticeable, particularly if you're warm, dehydrated, or have relatively little body fat.

This temporary change should not be confused with the progression of venous disease.

A vein that looks more prominent immediately after exercise is not necessarily a vein that has become structurally worse.

If the appearance returns toward normal after you cool down and recover, that is generally much less concerning than a persistent change accompanied by increasing swelling, skin changes, or pain.

What If Running Makes My Legs Hurt?

This is where things become more complicated.

Some people with varicose veins can run without any significant symptoms. Others notice that running makes their legs ache, feel heavy, or swell.

That doesn't necessarily mean running is damaging the veins. It may mean that you already have underlying venous insufficiency and the increased activity is exposing symptoms that are already there.

Venous disease can cause symptoms including aching, leg fatigue, heaviness, swelling, and discomfort. More advanced disease can cause skin changes and, eventually, venous ulcers.

For example, someone may feel fine during a short morning run but develop significant leg heaviness later in the day. Another person may notice swelling around the ankle after a long run.

Those symptoms are worth paying attention to.

The appropriate response isn't necessarily to stop exercising. Instead, it may be worthwhile to determine whether an underlying vein problem is contributing to the symptoms.

Running vs. Being on Your Feet All Day

There is an interesting difference between running and simply standing.

When you're standing still, your calf muscles aren't doing much to help pump blood upward. Gravity is continuously working against venous return, and blood can pool in the lower legs.

When you're running, your calf muscles are repeatedly contracting.

That's one reason movement is generally preferable to prolonged immobility for venous circulation. The National Heart, Lung, and Blood Institute recommends staying physically active and explains that walking and exercise cause the muscles of the lower legs to contract, helping blood return toward the heart rather than pool in the legs.

So if you're comparing a 30-minute run with 30 minutes of standing in one place, the run is generally much more beneficial for circulation.

The problem for some people isn't exercise itself. It's the underlying condition of the veins.

Can Running Cause Spider Veins?

There is also a common misconception that running causes spider veins.

Spider veins are small, visible blood vessels that appear close to the surface of the skin. They are different from larger varicose veins, although the two can occur together.

There is not strong evidence that running itself causes spider veins.

Genetics, hormones, age, pregnancy, and other factors play a much more important role.

If you've recently started running and suddenly notice more visible veins, it is easy to assume the two are connected. But the timing doesn't necessarily mean that the running caused the veins.

It may simply be that you're noticing them because you're paying closer attention to your legs—or because increased circulation and changes in body composition have made previously subtle veins more visible.

Should You Stop Running If You Have Varicose Veins?

For most people, having varicose veins alone is not a reason to stop running.

If running feels good and you're not experiencing significant symptoms, there is generally no reason to assume that you're harming your veins simply by continuing your normal running routine.

In fact, staying active can be an important part of managing venous disease.

The Society for Vascular Surgery lists walking and other lifestyle measures among approaches that can help people with varicose veins, while the National Heart, Lung, and Blood Institute similarly recommends physical activity to help blood move through the legs.

That said, your symptoms matter.

If running consistently produces significant pain, swelling, or heaviness, don't simply assume that you need to push through it.

Your body may be telling you that something deserves evaluation.

What Can You Do If Running Aggravates Your Symptoms?

If you have varicose veins and running seems to aggravate your symptoms, there are several things worth considering.

Reduce the intensity temporarily

You don't necessarily have to give up running altogether.

Try reducing your mileage, pace, or frequency for a period of time and see whether your symptoms improve.

A gradual return to your normal routine can sometimes make exercise more comfortable.

Walk or alternate walking and running

Walking is an excellent form of exercise for people with venous disease.

If continuous running causes discomfort, try alternating running with walking. You can still get substantial cardiovascular and muscular benefits while reducing the impact on your legs.

Pay attention to swelling

Mild, temporary changes after exercise can occur, but persistent or progressively increasing swelling deserves attention.

In particular, take note if one leg consistently becomes significantly more swollen than the other.

Elevate your legs after exercise

Elevating your legs while resting can help reduce pressure in the veins and may improve swelling and discomfort.

For some people, simply spending several minutes with the legs elevated after a workout can make a noticeable difference.

Consider compression

Compression garments can help some people with venous symptoms by reducing pooling and supporting venous return. They may be particularly useful for people who experience aching or swelling during prolonged activity.

However, compression isn't appropriate for everyone, and the type and amount of compression should be individualized.

If you have significant vascular disease or other circulation problems, talk with a healthcare professional before choosing compression therapy.

When Should a Runner Have Varicose Veins Evaluated?

If you have visible varicose veins but no symptoms, an evaluation may not be urgent.

However, it is worth talking with a vein specialist if you have persistent or worsening symptoms such as:

  • Leg aching or throbbing
  • A feeling of heaviness or fatigue
  • Swelling around the ankle or lower leg
  • Symptoms that consistently worsen throughout the day
  • Itching around the veins
  • Skin discoloration around the ankles
  • Skin that becomes thickened, hardened, or irritated
  • A varicose vein that becomes painful or tender
  • Symptoms that are interfering with running or other activities

One of the most useful things a vein evaluation can determine is whether the visible varicose veins are actually associated with venous reflux.

A duplex ultrasound can evaluate blood flow through the superficial and deep veins and identify abnormal reflux. Duplex ultrasound is an important component of the evaluation of patients with varicose veins and chronic venous disease.

That information matters because treating a visible vein without understanding the underlying source of the problem may not address the reason the veins developed in the first place.

What If I Have Varicose Veins but Want to Keep Training for a Race?

In most cases, having varicose veins does not automatically mean you need to abandon your training program.

If you're preparing for a half marathon, marathon, or another endurance event, the key is to pay attention to how your legs respond as your training volume increases.

A runner who has no symptoms and stable varicose veins may be able to continue training normally.

A runner who develops progressively increasing swelling, aching, heaviness, or other symptoms may benefit from a medical evaluation before simply increasing mileage.

This is especially important because runners sometimes dismiss leg symptoms as ordinary consequences of training.

Some soreness after a long run is normal.

Persistent heaviness, swelling, or aching that repeatedly follows the same pattern may be something else.

The Bottom Line: Running Usually Isn't the Enemy

If you have varicose veins, you don't necessarily have to choose between healthy exercise and healthy veins.

Running does not appear to be a direct cause of varicose veins, and regular physical activity is generally beneficial for venous circulation. The contraction of the calf muscles helps move blood out of the lower legs, and exercise is recommended as part of managing chronic venous disease.

The important distinction is between the appearance of your veins, your symptoms, and the underlying condition of your veins.

Running may temporarily make veins more noticeable. It may also aggravate aching or swelling in someone who already has venous insufficiency. But that doesn't necessarily mean running is making the underlying vein disease worse.

If you're a runner and your varicose veins are becoming increasingly symptomatic—or if leg swelling, heaviness, pain, or skin changes are beginning to interfere with the activities you enjoy—it may be time to have your veins evaluated.

The goal isn't necessarily to get you to stop running.

The goal is to understand what's happening in your veins so you can keep doing the things you enjoy with healthier, more comfortable legs.
